Terms of Service

1. Service description
Mairi (the “Service”) provides patient-facing Personal Health Record (PHR), AI symptom check, drug scan, and visit-prep QR functionality (Phase 1 scope).
2. Account
- Users create accounts in the Service and may cancel anytime.
- You are responsible for safeguarding your credentials.
- Report any unauthorised access promptly to support@mairi.health.
3. Subscription
- Monthly and annual auto-renewing subscriptions are offered.
- Cancellation procedures are detailed in the Commercial Transactions Act Notice.
- Refunds follow the policies of each store (App Store, Google Play, Stripe).
4. Prohibited conduct
You agree not to:
- Attempt unauthorised access, reverse-engineering, or unauthorised reproduction of content
- Impersonate medical professionals or public authorities
- Obtain other people’s personal information unlawfully
- Use the Service to commit illegal acts
5. AI output
- AI output from the Service is reference information only and not a medical diagnosis.
- Call emergency services (119 in Japan) for any acute condition.
- Instructions from physicians, pharmacists, and nurses always take precedence over the Service’s output.
6. Intellectual property
- Content, design, and software in the Service belong to crealize.llc.
- Data you upload or enter is owned by you.
7. Disclaimer
- To the extent permitted by law, crealize.llc is not liable for damages arising from your use of the Service.
- Responsibility for medical outcomes rests with the treating physician.
8. Changes
We may update these Terms periodically. Material changes will be announced via the app and email.
9. Governing law
- Governing law: laws of Japan
- Exclusive jurisdiction: Tokyo District Court
10. Termination
If you breach these Terms, crealize.llc may suspend or delete your account without prior notice.
Mairi is not a medical device (SaMD under Japan's PMD Act). Consult a licensed clinician for diagnosis, treatment, or prescription.
